# Real GLP-1: more to measure than weight

> Real GLP-1: beyond the weight-loss headline — Real GLP-1 follows semaglutide research beyond the scale, through appetite, side effects, and the heart and kidney results that widened the story.

**Semaglutide, up close**

The scale gets the headline. The findings beyond it get equal billing in my mind: appetite, uncomfortable side effects, and heart and kidney outcomes.

A change in weight can be dramatic. It also makes a very easy headline. My interest begins with what happens when the scale moves to the edge of the picture and there is room to ask about everything around it.

Semaglutide is a [GLP-1 analogue](https://pubmed.ncbi.nlm.nih.gov/39728000/), a molecule modelled on GLP-1. The name of this site reflects an ambition I have for the conversation around it: give the less photogenic findings as much attention as the headline result. Appetite, discomfort and an outcome such as a stroke deserve different kinds of attention.

## The weight-loss result

There is a substantial result to begin with. In STEP 1, adults with overweight or obesity without diabetes had a [mean body-weight change of -14.9% with semaglutide versus -2.4% with placebo at 68 weeks](https://pubmed.ncbi.nlm.nih.gov/33567185/). That finding seems substantial to me. The scale earned its place in the story.

An average is a useful account of a group. Turning it into a personal forecast asks it to do a different job. The [weight-loss essay](/weight-loss) spends time with the result itself, including the people studied and the comparison that gives the number its meaning.

## Clinical outcomes

SELECT widened my interest. Adults with preexisting cardiovascular disease and overweight or obesity, without diabetes, had [fewer major cardiovascular events with semaglutide than with placebo](https://pubmed.ncbi.nlm.nih.gov/37952131/). That finding redirected my attention from how much a body weighed to what happened to it.

FLOW then supplied a separate result in a separate population: [fewer major kidney-disease events in people with type 2 diabetes and chronic kidney disease](https://pubmed.ncbi.nlm.nih.gov/38785209/). My interest grows as these trials open different questions about semaglutide. A kidney outcome and a weight measurement each describe something worth knowing. Neither can do the other’s work.

## Essays

- [Semaglutide weight loss](/weight-loss): the STEP 1 comparison, with its population attached.
- [Appetite and the brain](/appetite): a surprising rodent experiment, an ordinary lunch, and an early alcohol trial.
- [Heart and kidney outcomes](/outcomes): the clinical events and physical limitations that broadened my interest.
- [Semaglutide side effects](/side-effects): why an encouraging overall result still leaves room for discomfort and specific cautions.

On Real GLP-1, my interest in semaglutide’s published results runs from the scale to the less photogenic outcomes, and these essays offer no medical advice or treatment service.
